It should be understood that the filing of this report is considered to be an official Police Report and that providing false or misleading information may be considered as filing a false report and may subject the person making the report to criminal charges.

All reports MUST be filled out by the complainant only and no “second hand” or “hearsay” information will be accepted. If there are any witnesses to the alleged incident, those individuals should be listed on the report, as they may be requested to fill out a separate form subject to the same procedures. All reports must be filled out independently to make certain that the information presented is factual and not compromised.

Once completed, the reports will be reviewed by the Chief of Police, or if the complaint is against the Chief, by the City Administrator, who will then conduct or have conducted an internal investigation into the matter. When the investigation is completed the complainant will be notified of a finding. In the event that the Chief of Police believes that it is inappropriate for the LeRoy Police Department to investigate the matter, another agency such as Illinois State Police or McLean County Sheriff's Department may be requested to conduct the inquiry. In the event that the outside assistance is requested, persons reporting alleged misconduct will be required to cooperate in order to facilitate a fair and impartial inquiry.

Upon request, results of said investigation may be given to the complainant, but be advised that any discipline resulting from any complaint and investigation, will not be disclosed to the complainant as it is not permitted by Illinois State Law.
